Social Worker/Therapist - San Jose (Hybrid)

As the Therapist you will be responsible for maintaining case records and progress notes, facilitate family and individual client meetings, and provide client behavioral intervention as needed. The Therapist on the team coordinates services and treatment planning for youth; as a result, your primary role is to ensure that all parties are working together toward stability for the youth and family. Team members provide services almost exclusively in the community: in families’ homes, schools, other community locations and have the opportunity to complete documentation from home.

ABOUT KAISER WRAPAROUND
This Kaiser Wraparound Program is a partnership between Kaiser Permanente and Seneca Family of Agencies, providing behaviorally focused mental health services to children and families. The program aims to provide children and their families with the support and services they need for children to maintain the lowest level of care possible and avoid out of home placements (group homes, juvenile hall, and hospitalization).

Responsibilities

  • Meet with clients and caregivers on a weekly basis to further treatment goals and provide psychoeducation, coordinate care between providers etc.
  • Develop Child and Family teams made up of natural supports for the purpose of treatment planning and sustainable support.
  • Cultivate and maintain engagement and alignment with community members, Kaiser partners and other providers.
  • Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate.
  • Identify engagement needs for each youth and family as needed and work with Wraparound team members to build connections for family.
  • Chart mental health notes, assessments, treatment plans, outcome measures, incident reports, and other documentation accurately and in a timely manner.
  • Function as team clinical leader for respective caseload, and uphold the principles of the wraparound program.
  • Participate in individual and/or group supervision.

Qualifications

Required:
  • Master’s degree in Counseling/Psychology/Social Work
  • Registered with BBS as: ASW, AMFT, APCC, or full licensed 
  • Flexible schedule with ability to work some evenings and weekends
  • Be part of a rotating emergency pager system, scheduled ahead of time
  • Must be at least 21 years of age
  • Own vehicle, Valid California driver's license and insurability with Seneca's automobile policy
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
  • Must show proof of COVID-19 vaccination
Preferred:
  • Bilingual Spanish skills
  • Experience working with children in out-of-home care
  • Excellent organizational and critical-thinking skills

Schedule

  • Full-time
  • Mon - Fri; 9AM - 5PM OR 10AM - 6PM
  • Flexible schedule with ability to work some evening and weekend
  • Hybrid; provide in-person services & complete documentation remotely

Benefits

  • Starting at $80,000 - $94,000 annually, actual salary dependent on creditable experience above the minimum qualification
  • Additional $4000 salary increase for BBS licensure
  • An additional $6000 per year upon passing a bilingual language proficiency exam
